I, Kellye Strickland, declare under penalty of perjury that the following is true and
correct to the best of my knowledge and belief:
- I am the Plaintiff in the above-captioned matter, and I submit this declaration in
support of my civil rights complaint filed in the United States District Court for the
District of Minnesota.
- I am a resident of Tempe, Arizona, and I am permanently and totally disabled. I have
received Social Security Disability Insurance (SSDI) benefits for nearly twenty years due to
diagnoses of PTSD and Bipolar Disorder. These conditions were disclosed repeatedly to the
defendants through formal court filings and prior correspondence.
- I have personally reviewed all the factual statements in my complaint, including the
chronology of events, the descriptions of contact with court personnel, and the attached
exhibits labeled A through E.
- Each statement of fact made in the complaint and supported by attached documentation,
including records of improper service, administrative inaction, denial of access to hearings,
and unequal treatment of similarly situated parties—is true and correct to the best of my
knowledge, memory, and belief.
- The exhibits attached to my complaint are authentic copies of the original documents,
which I either received directly from the court, obtained through formal records requests,
or generated through properly filed motions.
- I make this declaration voluntarily and in good faith, in support of my federal civil
rights claims under 42 U.S.C. § 1983, the Fourteenth Amendment, and Title II of the
Americans with Disabilities Act.
I declare under penalty of perjury under the laws of the United States of America that
the foregoing is true and correct.
Respectfully submitted,
Dated:
